I wonder if alcohol was a contributory factor?
Commercial vessel should have all the bells and whistles and operational processes to prevent this, but maybe it was a mechanical fault....

Not a fishing boat but a fish-carrier (for fish farms). The nature of their work does require them to operate in quite shallow water sometimes.
